Job summary

Trilogy Health Services in Evansville, IN is seeking a licensed Practical Nurse to provide compassionate, holistic care on assigned units. Visit residents daily to monitor condition, document thoroughly, and coordinate with charge nurses and physicians as needed.

You’ll assist with medications, maintain charts, and participate in safety procedures to ensure resident well-being. State LPN license and CPR certification are required.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current CPR certification required.

Responsibilities

• Visit residents daily to observe and evaluate health status and notify nurse of changes.
• Identify problems and emergencies and initiate lifesaving measures if physician is unavailable.
• Provide holistic care within the LPN scope.
• Monitor inventory of medications, supplies and equipment to meet resident needs.
• Complete medical forms, reports, evaluations and charting as needed.
• Report occupational exposures to blood and other hazards per policy.
