Job summary

Trilogy Health Services, LLC in South Bend, IN is seeking a Licensed Practical Nurse (LPN) to provide direct clinical care and supervise nursing assistants on the unit. You will visit residents daily, monitor physical and emotional status, respond to changes or emergencies, assist with medications, perform administrative duties, and ensure thorough charting.

A current LPN license and CPR certification are required.

Qualifications

• Must have and maintain a current, valid state LPN license.
• Current, valid CPR certification required.

Responsibilities

• Visit residents on assigned unit daily to observe and evaluate status and notify changes to the charge nurse or physician.
• Identify problems and emergencies and initiate lifesaving measures when physician is unavailable.
• Provide holistic care within the LPN scope.
• Monitor medications, supplies, and equipment and ensure adequate stock.
• Perform administrative duties such as forms, reports, and charting as needed.
• Report occupational exposures according to facility policies.
