Highland Oaks Health Center is seeking a compassionate Licensed Practical Nurse (LPN) to join our team in a long-term care setting. The LPN will provide direct resident care, administer medications, monitor conditions, and collaborate with an interdisciplinary team to ensure high-quality care.

Qualifications include a current LPN license in good standing and strong communication skills. We value dedication, reliability, and a patient-centered approach to care.

Qualifications

• Current LPN license in good standing.
• CPR certification preferred.
• Long-term care experience preferred but not required.
• Strong communication, organizational, and critical-thinking skills.

Responsibilities

• Provide skilled nursing care in accordance with physician orders and facility policies.
• Administer medications and treatments accurately and safely.
• Monitor residents for changes in condition and communicate concerns appropriately.
• Complete timely and accurate documentation.
• Develop and update resident care plans.
• Supervise and support nursing assistants and other care staff.
• Promote resident dignity, independence, and quality of life.
• Ensure compliance with state, federal, and facility regulations.
• Participate in admissions, transfers, and discharge processes.
• Respond appropriately to emergencies and resident needs.
