Job summary

Trilogy Health Services, LLC is hiring an Licensed Practical Nurse (LPN) at our Kingston Health Center of Perrysburg, OH. You will provide direct clinical care to residents and supervise nursing assistants on your unit.

Responsibilities include daily resident assessments, timely reporting of changes, medication and supply monitoring, and completing required forms. CPR certification and an active LPN license are required.

Qualifications

• Must have and maintain a current, valid state LPN license
• Current, valid CPR certification required

Responsibilities

• Visit residents daily to assess physical and emotional status and notify charge nurse/physician of changes
• Identify problems and emergencies and initiate lifesaving measures when physician is unavailable
• Provide holistic care within the LPN scope
• Monitor medications, supplies, and equipment inventory to ensure adequate stock
• Complete medical forms, reports, evaluations, and charting as required
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals per policy
