Qualifications

• Active LPN license in Massachusetts
• BLS (CPR) certification required
• Ability to care for multiple patients in a fast-paced setting
• Strong clinical judgment and communication skills

Responsibilities

• Provide patient care per standards, plan of care, and physician’s treatment
• Implement individualized care plans with Nurse Manager and team
• Administer medications per POC and facility policies
• Monitor residents' conditions and report changes promptly
• Maintain accurate documentation per orders and care plans
• Support a safe, respectful, and therapeutic environment for residents and families
