Hallmark Healthcare of Pekin is seeking an LPN to provide compassionate, patient-centered nursing care to residents in Pekin, IL. The role includes medication administration, collaboration with the care team, and documentation within facility standards.

Qualifications

Illinois LPN license required.
Graduate of an accredited nursing program.
Compassionate, patient-focused care.

Responsibilities

Provide compassionate, high-quality nursing care to residents
Administer medications and treatments within your scope of practice
Collaborate with interdisciplinary healthcare team members to support quality resident outcomes
Monitor resident conditions, including vital signs, and report changes appropriately
Encourage restorative care to support resident independence and well-being
Maintain accurate and timely documentation of all care provided
Support and mentor nursing staff to foster a positive, resident-centered environment
Ensure resident comfort, dignity, safety, and emotional well-being
Follow facility standards, policies, and nursing best practices
Perform additional duties as assigned by management in a resident-centered care environment
