Qualifications

• Graduate from an accredited LVN/LPN program with a valid state license.
• Two or more years of acute care or rehabilitation experience preferred.
• ACLS certification preferred; CPR certification required.

Responsibilities

• Supervises CNAs under direction of RN.
• Collects patient care data and monitors conditions.
• Contributes to care and discharge planning.
• Implements care per plan and collaborates with health team.
• Evaluates outcomes and revises care as needed.
• Participates in orientation and education of personnel.
• Documents care accurately and adheres to policies.
• Performs medication administration and patient rounds.
• Ensures diet and fluid orders are followed.
• Maintains timely and complete nursing documentation.
