Job summary

360 Community Services in Omaha, NE is seeking a Licensed Practical Nurse (LPN) for a full-time, on-site role. You will monitor health status, administer medications, document care, and collaborate with interdisciplinary teams to support person-centered care plans in residential and community settings.

You will conduct health assessments, educate individuals and staff on wellness and preventive care, and ensure adherence to clinical protocols while coordinating with families and healthcare

Qualifications

• Active LPN license in Nebraska and CPR/First Aid certification required.
• Direct patient care, medication administration and basic clinical procedures in residential or community settings.
• Accurate health assessments, detailed documentation, and adherence to care plans and protocols.
• Strong communication and interpersonal skills for working with individuals, families and teams in a person-centered environment.
• Knowledge of mental health, developmental disabilities or behavioral health supports; experience in similar settings preferred.
• Ability to prioritize tasks, manage time and respond calmly in urgent or crisis situations.
• On-site work with occasional lifting and assistance with activities of daily living.
• High school diploma or equivalent; completion of accredited LPN program required.
