Required Education and Experience for Licensed Practical Nurse

• Completion of an accredited practical nursing program.

Required Certifications and Licenses for Licensed Practical Nurse

• Licensed as a Licensed Practical Nurse (LPN) in the State of Oklahoma or able to obtain within 90 days of hire date in the position.
• Registered as a Licensed Practical Nurse in the State of Oklahoma.
• Basic Life Support (BLS).
• Cardiopulmonary Resuscitation (CPR) certifications.

Required Education and Experience for Senior Licensed Practical Nurse

• Completion of an accredited practical nursing program.
• 24 months experience as a Licensed Practical Nurse.

Required Certifications and Licenses for Senior Licensed Practical Nurse

• Licensed Practical Nurse (LPN) in the State of Oklahoma.
• Registered as a Licensed Practical Nurse in the State of Oklahoma.
• Basic Life Support (BLS) certification.
• Cardiopulmonary Resuscitation (CPR) certification.
