Job summary

You will deliver skilled nursing, educate families, administer medications, document care, and travel within the service area to reach patients where they are.

Qualifications

• Complete LPN program accredited by the National League for Nursing (NLN).
• Minimum of one year of hands-on clinical LPN experience.
• Current Michigan LPN license in good standing.
• NAPNES certification or pharmacology exam completion.
• Reliable vehicle for daily travel to patient homes.
• Valid Michigan driver's license and satisfactory driving record.

Responsibilities

• Deliver skilled nursing care in patients' homes under the direction of the Registered Nurse and care team.
• Monitor patient conditions and report changes to the appropriate healthcare providers.
• Administer medications and treatments as ordered.
• Educate patients and family members on disease management and care plans.
• Complete accurate and timely documentation.
• Travel throughout the assigned service area to provide patient care.

Schedule

• Full-Time
• Monday-Friday
• 8:00 AM - 4:30 PM
• 1 Saturday a month.
