Qualifications

• Licensed Practical Nurse (LPN) with behavioral health care experience.
• Experience with non-face-to-face care management and clinical documentation.

Responsibilities

• Review resident medical records, diagnoses, medications, behavioral health notes, lab results, and condition changes.
• Document validated behavioral health screenings (PHQ-9, GAD-7, MoCA, BIMS) and monitoring tools.
• Monitor behavioral health symptoms and treatment response; track progress toward goals.
• Develop and update individualized behavioral health care plans with the care team.
• Coordinate behavioral health services and follow-up recommendations; communicate findings to providers and facility staff.
• Prepare case summaries for psychiatric consultation and track recommendations and outcomes.
• Maintain accurate CoCM registry and documentation; ensure timely, compliant charts and billing records.
