Infinite Medical P.C. is seeking a dedicated Licensed Practical Nurse (LPN) to serve as a Behavioral Health Care Manager within our Collaborative Care Model.

This in-office, non-face-to-face care management position supports residents in skilled nursing and long-term care facilities through medical record review, behavioral health screening, care planning, registry management, and team collaboration. Competenze

• Active and unrestricted LPN license.
• Graduate of an accredited practical nursing program.
• Strong clinical documentation and medical-record review skills.
• Ability to escalate behavioral health, medication, and safety concerns.
• Proficient with EMRs and time management in high-volume settings.

Mansioni

• Review patient records and monitor behavioral health documentation.
• Document PHQ-9, GAD-7, MoCA, BIMS assessments.
• Maintain non-face-to-face contact with patients and care team.
• Develop individualized behavioral health care plans.
• Coordinate behavioral health services and follow-up.
• Prepare patient case summaries for psychiatric review.
• Track enrollment, screening results, and care-plan updates.
• Ensure HIPAA and documentation standards compliance.
• Escalate urgent concerns per protocol.
