Job summary

Lakewood Health System is seeking a part-time LPN/CMA to join the Care Management Team in central Minnesota. The role focuses on patient navigation, care coordination, and proactive engagement to improve health outcomes. You’ll work closely with providers to maintain care plans and support CCM/HCH initiatives.

The ideal candidate holds Minnesota LPN licensure or CMA/AAMA certification, plus BLS certification, and demonstrates strong EMR proficiency, organization, and teamwork.

Qualifications

  • Active Minnesota LPN licensure or current CMA/AAMA certification.
  • Current BLS Healthcare Provider certification.
  • Strong clinical and patient-care skills within scope of practice.
  • Proficiency in EMR documentation and workflow management.
  • Ability to prioritize tasks, problem-solve, and work independently.
  • Strong organizational skills and attention to detail in documentation and reporting.
  • Effective communication and collaboration with patients, providers, and healthcare teams.

Responsibilities

  • Assess patients within applicable scope of practice and monitor status.
  • Maintain accurate care plans and timely EMR documentation.
  • Support patient rooming, screenings, intake, scheduling and care coordination.
  • Monitor calls, MyChart messages and provider in-baskets and respond appropriately.
  • Assist patients with navigating appointments, referrals and follow-up care.
  • Collaborate with providers and the care team to support continuity of care.
  • Support CCM and HCH initiatives and maintain documentation of activities.
