Job summary

Phoenix Home Care & Hospice in Independence, MO seeks an experienced LPN to advance into a Clinical Manager role. You will lead the Home Health team, support field staff, coordinate patient care, and address clinical challenges to ensure exceptional care.

You will help guide clinical operations, mentor nurses, and drive quality and compliance across visits and care plans, while collaborating with physicians and families.

Qualifications

• Missouri or Compact LPN license is required.
• Experience in nursing with Home Health preferred.
• Strong communication, organization and leadership abilities.
