Job summary

Phoenix Home Care & Hospice seeks a Licensed Practical Nurse (LPN) for home health in Springfield, MO. The role features a Monday–Friday day schedule with some weekend rotation, delivering in-home nursing care, assessments, education, and collaboration with the care team.

You must hold a Missouri or Compact LPN license, have a valid driver's license, and bring strong assessment, communication, and organizational skills. Competitive pay and benefits accompany the position.

Responsibilities

• Deliver skilled nursing care in patients' homes according to physician orders and individualized care plans.
• Perform comprehensive patient assessments and monitor changes in condition.
• Educate patients and families to promote confidence and independence at home.
• Collaborate with RNs, Physical Therapists, Occupational Therapists, Speech Therapists, physicians, and the interdisciplinary care team.
• Maintain accurate documentation using our AI-supported charting technology.
• Participate in quality improvement initiatives and ongoing professional development.

What You'll Do

Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with a weekend rotation approximately every 4 to 6 weeks.
