Marquis Home Care in New York is seeking an LPN Clinical Scheduler to coordinate and schedule patient visits for assigned clinicians and territories. You will ensure visits align with physician orders, plans of care, and regulatory requirements while collaborating with care teams.

The role emphasizes scheduling accuracy, effective communication with patients and clinicians, and maintaining up-to-date EHR data in a fast-paced home health environment.

Qualifications

• Current, active, and unrestricted New York State Licensed Practical Nurse (LPN) license required.
• Minimum one year of nursing experience required.
• Minimum of 6 months of home health care experience preferred.
• Knowledge of Medicare Home Health regulations, home care operations, and clinical visit requirements preferred.
• Strong organizational, time management, and multitasking skills.
• Excellent communication, customer service, and problem-solving abilities.
• Proficiency with Electronic Health Records (EHR) systems and scheduling software (Home Care Home Base).
• Working knowledge of computers including Outlook, Word, Excel, Teams.
• Ability to work independently and collaboratively in a fast-paced environment.
• Knowledge of medical terminology required.
• Experience with Homecare Homebase (HCHB) or comparable home health software preferred.

Responsibilities

• Coordinates and schedules all patient visits for assigned clinicians and territories.
• Assigns visits based on patient needs, clinician competencies, clinical scope of practice, geographic considerations, and productivity expectations.
• Ensures visit frequencies are scheduled according to physician orders and approved plans of care.
• Coordinates Start of Care (SOC), Resumption of Care (ROC), Recertification, Routine, Supervisory, and Discharge visits.
• Monitors clinician schedules daily and adjusts assignments to address admissions, urgent visits, cancellations, staffing changes, and patient needs.
• Utilizes clinical judgment to identify and elevate scheduling concerns that may impact patient safety, continuity of care, or regulatory compliance.
• Collaborates with clinical management staff to ensure timely completion of all ordered visits.
• Communicates effectively with patients, caregivers, clinicians, physicians, referral sources, and agency staff regarding scheduling matters.
• Maintains accurate scheduling information within the Electronic Health Record (EHR) system.
• Documents scheduling changes, communications, and visit coordination activities according to agency policies and procedures and directly with clinicians.
• Assists in monitoring clinician productivity, caseload distribution, and territory coverage.
• Coordinates staffing coverage for clinician absences, vacations, holidays, and emergency situations.
• Maintains confidentiality of all patient, employee, and agency information.
• Provides exemplary customer service to all internal and external customers.
• Participates in departmental meetings, performance improvement activities, and training programs as required.
• Ensures compliance with agency policies, Medicare Conditions of Participation, New York State Department of Health regulations, HIPAA requirements, and other applicable regulatory standards.
• Performs additional duties, responsibilities, and special projects as assigned by the Director of Patient Services for the smooth operation of the agency.
