Hillcrest Country Estates, part of Hillcrest Health & Living, seeks an experienced Clinical Care Coordinator (LPN) in Papillion, NE. Lead care coordination for post-acute patients, mentor staff, and ensure quality clinical outcomes.

The ideal candidate has Nebraska LPN licensure, BLS, and 2+ years in skilled/post-acute care with supervisory experience. This full-time role emphasizes clinical leadership and collaboration with an interdisciplinary team.

Qualifications

• Nebraska LPN license required.
• Bachelor’s degree in Nursing preferred.
• At least two years of experience in skilled nursing, acute care, or post-acute care required.
• At least two years of supervisory experience preferred.
• American Heart Association BLS certification required.
• RAC-CT certification preferred.

Responsibilities

• Provide clinical leadership and oversight for post-acute patients from admission to discharge.
• Coordinate care and staffing for daily operations.
• Conduct rounds and ensure follow-up for identified needs.
• Monitor quality, safety, and regulatory compliance.
• Review medication records for accuracy and compliance.
• Coach and mentor nursing team members.
• Participate in interdisciplinary team meetings and improve clinical pathways.
• Support safe staffing and direct patient care when needed.
