Job summary

The CCMA will participate in IDG meetings, coordinate communications with physicians, and uphold documentation standards while providing compassionate, professional patient-family interactions within hospice care

Qualifications

• Currently licensed in Florida as an LPN.
• Minimum of one year of nursing experience.
• Experience with EMR systems and proper documentation procedures.

Responsibilities

• Enter patient status changes into EMR in a timely manner.
• Keep team census spreadsheet up-to-date.
• Prepare daily status reports.
• Assist in IDG meetings and distribute agenda.
• Notify team members and outside physicians of patient status changes.
• Print and distribute reports and schedules as needed.
