Qualifications

  • Minimum of 1 year nursing experience.
  • Hospice/Oncology/Home Health experience preferred.
  • 1 year acute care nursing with training in symptom management.
  • Valid Florida driver’s license and auto insurance.

Responsibilities

  • Assess physical, psychological, social, and spiritual needs of patient and family and initiate intervention.
  • Take independent action to identify and address needs.
  • Collaborate with Team RNs and care team to maintain quality care.
  • Assist in orientation of new staff and mentor teammates.
  • Educate community about hospice care and services.

Description

POSITION SUMMARY:

Reporting to the Clinical Care Manager, Crisis Care LPN is committed to providing the highest quality of care as identified in the Gulfside Healthcare Services Mission. Primary role will be to provide crisis care clinical services for appropriate patients as assigned, as well as visit home and facility team patients as needed. This role identifies and coordinates patient and family needs, providing preventative, palliative, and supportive care to terminally ill patients and their families. This role will collaborate with the Team RNs and other disciplines to ensure the highest quality of care is maintained.

EDUCATION AND QUALIFICATIONS:

  • Currently licensed in the State of Florida as Licensed Practical Nurse (LPN).
  • Must maintain current American Heart Association approved CPR certification.
  • Minimum of one (1) year nursing experience.
  • Hospice, Oncology, or Home Health experience is preferred.
  • Minimum of One (1) year acute care nursing experience with training in Symptom Management.
  • Most possess and maintain a current Florida driver’s license.
  • Most possess and maintain automobile insurance coverage.

ESSENTIAL JOB RESPONSIBILITIES:

  • Perform ongoing assessment of physical, psychological, social, and spiritual needs of the patient and family unit and initiate appropriate intervention.

  • Educates the community regarding hospice care, philosophy, and concept through individual consultation with people in the community.
  • Communicate regularly with team RNs and Clinical Care Manager.
  • Maintain current with organizational and departmental communication via e-mail. All clinical staff are required to check their e-mail accounts at the start of each shift.
  • Effectively communicate, collaborate, and coordinate with colleagues and management on issues related to nursing process and service.
  • Work with Team RN to create and implement nursing care plans which are reviewed daily and updated as
  • Consider financial implications in rendering cost-effective nursing
  • Ensure professional standards of nursing practice are
  • Complete required documentation prior to departing from scheduled
  • Actively participate in the IDG (Interdisciplinary Group) and communicate patient needs appropriately to other disciplines when requested
  • Collaborate with team physician daily on the status of the
  • Performs other duties as requested by the Clinical Care
