Job summary

Amberwell Health Atchison is seeking a licensed practical nurse (LPN) to support onsite providers and the care team in the clinic. The LPN will work with the EMR system to complete nursing tasks, communicate with patients, and coordinate follow-up testing and referrals as part of a patient-centered care team.

The role requires an active Kansas nursing license and a 2-year degree in nursing, with BLS certification. Prior clinic experience is preferred.

Qualifications

• Completion of accredited Vocational/Technical Nursing Program.
• Active Kansas nursing license.
• BLS certification.
• 1–2 years clinic experience preferred.

Responsibilities

• Assist the care team in nursing tasks through the EMR.
• Communicate results and education to patients via phone or portal and document in the EMR.
• Perform medication prior authorizations.
• Document patient/family interactions, medications dispensed, and test results in the medical record.
• Coordinate and manage follow-up testing and referrals with the provider.
• Monitor patients regularly to promote wellness and chronic care management.
• Provide patient education materials and relay provider instructions.
