Signature HealthCARE of Norfolk in Virginia seeks a dedicated LPN/LVN to provide direct nursing care, assist with ADLs, and supervise nursing staff on the 3p–11:30pm shift. The role includes medication administration, wound care, and coordinating patient care with the interdisciplinary team.

Applicants should hold an active LPN/LVN license, with preferred experience in long‑term care or supervision; CPR/BLS preferred. Excellent documentation and communication skills are essential.

Qualifications

• Active LPN/LVN license in the state of practice.
• Experience in long‑term care or supervisory roles desired.
• Strong clinical observation and documentation skills.

Responsibilities

• Provide direct nursing care including medication administration and wound care.
• Assist residents with ADLs ensuring comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents' status and report changes to physicians.
• Develop and review resident care plans with the interdisciplinary team.
• Complete clinical documentation per policy.
• Coordinate admissions, transfers and discharges.
• Participate in quality assurance and regulatory surveys.
• Perform other duties as assigned by the Director of Nursing.
