Job summary

Signature HealthCARE's McCreary County Rehab & Wellness Center in Kentucky is seeking an LPN to provide direct nursing care, administer medications, monitor residents, and ensure comfort and safety. You will supervise nursing assistants, document care, and coordinate admissions and discharges within a collaborative team.

The ideal candidate has an active LPN/LVN license, 1+ year of clinical nursing experience (LTC preferred), and strong EHR/documentation skills.

Qualifications

Current, active LPN/LVN license in the state of practice.
CPR/BLS certification preferred (American Heart Association or American Red Cross).
Minimum one year of clinical nursing experience; long-term care experience preferred.
Experience in a supervisory or charge nurse role preferred.
Strong clinical observation, medication administration, and documentation skills.
Knowledge of Federal and State long-term care regulations.
Effective verbal and written communication skills.
Familiarity with electronic health records (EHR) systems.

Responsibilities

Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations.
Monitor and observe residents’ physical and emotional status; notify physician and responsible party of changes.
Develop, implement, and review individualized resident care plans with the interdisciplinary team.
Complete accurate and timely clinical documentation per facility policy and procedure.
Coordinate resident admissions, transfers, and discharges per procedures.
Participate in QA activities, staff education, and regulatory surveys as needed.
Perform other duties as assigned by the Director of Nursing or designee.
