Signature HealthCARE in Terre Haute, IN, is seeking a dedicated LPN/LVN to provide direct nursing care to residents, administer medications, and support daily living activities. You will supervise nursing assistants, monitor residents, and collaborate with an inter-disciplinary team to ensure high-quality, person-centered care.

Formación

• Current, active LPN/LVN license in Indiana.
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role preferred.
• Strong observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise clinical judgment.
• Familiarity with electronic health records (EHR) systems.

Responsabilidades

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs while ensuring comfort, dignity, and safety.
• Supervise and direct nursing assistants and medication aides per policy.
• Monitor residents’ status and notify physicians of changes.
• Develop and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation per policy.
• Coordinate admissions, transfers, and discharges.
• Participate in quality assurance activities and staff education.
• Perform other duties as assigned by the Director of Nursing.
