Job summary

Signature HealthCARE is seeking a dedicated LPN/LPN to provide direct nursing care at our facility. You will administer medications, perform wound care, and support residents with daily activities while maintaining high clinical documentation standards.

The role also includes supervising nursing staff during shifts and collaborating with the interdisciplinary team to update care plans, ensuring safety and quality of life for residents.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred.
• Minimum 1 year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role is a plus.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with ADLs to ensure comfort, dignity and safety.
• Supervise nursing assistants and medication aides during shifts.
• Monitor residents and notify physician of any changes in condition.
• Develop and review individualized resident care plans with the interdisciplinary team.
• Complete accurate and timely clinical documentation per policy.
