Job summary

Azalea Health & Rehab – Metter, GA is seeking an LPN Charge Nurse for a full-time day shift (7a-7p). The role involves supervising CNAs, administering medications, and delivering high-quality patient care within a compassionate team environment.

You will collaborate with RNs and the care team to monitor residents and document care plans, while upholding regulatory standards and providing patient-centered service.

Qualifications

• Active LPN license (Georgia or eligible)
• Strong clinical judgment and teamwork required
• Excellent communication and reliability in patient care

Responsibilities

• Administer medications and treatments as prescribed
• Monitor residents' conditions and report changes to the care team
• Provide direct nursing care in accordance with care plans
• Supervise and support CNAs in delivering quality care
