Job summary

Lynn Haven Health & Rehab in Georgia is seeking a dedicated Licensed Practical Nurse to join our compassionate interdisciplinary team. You will administer medications, monitor resident conditions, and provide direct, patient-centered care in a supportive environment.

You’ll work with RNs, CNAs and other care professionals, ensuring accurate documentation and adherence to all policies. A GA LPN license or eligibility is required; competitive wages and comprehensive benefits are offered.

Qualifications

• Active LPN license (State of Georgia) or eligible.
• Experience in long-term care is a plus.
• Excellent communication and teamwork skills.

Responsibilities

• Administer medications and treatments as prescribed.
• Monitor residents’ conditions and report changes to the care team.
• Provide direct nursing care in accordance with care plans.
• Supervise and support CNAs in delivering quality care.
• Maintain accurate and timely documentation and ensure policy compliance.
