Qualifications

• Nursing diploma required (assoc. or bach. degree).
• Current active LPN license in applicable state.
• 1 year clinical post-acute care experience preferred.
• Prior case management, utilization review, and discharge planning experience preferred.

Responsibilities

• Generate written communication that is clear, concise, and well-organized.
• Excellent organizational skills and be efficient in prioritizing and managing time and assignments.
• Contribute to organizational goals and support strategic plans.
• Expert knowledge in field of practice.
• Make independent decisions when warranted.
• Know laws, regulations, and guidelines governing post-acute care.
• Implement and interpret department programs, goals, and procedures.
• Maintain professional relationships and confidentiality.
• Follow Life Care policies and complete compliance training.
• Demonstrate efficient use of computer systems.
• Serve as liaison to external case managers, family, physicians, and community resources.
• Train and educate patients, families, associates, and other providers of care.
• Coordinate care according to standards of practice and ethical performance.
• Exhibit excellent customer service and positive attitude toward patients.
• Attend to evacuation procedures and safety duties.
• Communicate effectively in an interdisciplinary team.
• Physical duties include sitting, standing, lifting, and mobility as needed.
