Qualifications

• High School Diploma or GED required.
• Graduation from an accredited LPN program with PA license to practice.
• At least two years of care coordination or community health experience.

Responsibilities

• Provide service coordination including case management and goal-oriented action plans for patients.
• Maintain interactions with patients and families to support adherence to treatment plans and self-management.
• Act as liaison for patients regarding medical and dental benefits.
• Monitor care delivery by documenting progress and communicating with the care team.
• Ensure HIPAA compliance and protect confidential information.
• Collect data and prepare monthly reports.
