Job summary

Qualifications

• Graduate of approved school of nursing.
• Current Massachusetts license as an LPN in good standing.
• Excellent English language skills, verbal and written.
• Must be professional, proactive, collaborative and results-oriented.
• Solid analytical and organizational skills.
• Ability to innovate, think strategically and handle multiple priorities.
• Willingness to learn and adapt to change.
• Computer literacy required.

Responsibilities

• Ensure care delivery aligns with safe nursing practice and core competencies.
• Collaborate with MD, NP, nurses, social worker, therapists, and dietician to meet patient needs.
• Implement evidence-based interventions to optimize patient outcomes.
• Operate in a primary nursing model with RN oversight of assessment and care planning.
• Participate in care planning and support patient autonomy and choice.
• Document care in the electronic medical record and ensure accuracy.
• Escalate safety concerns and initiate required reports (DPH, incidents).
• Delegate and supervise nursing assistants and participate in performance improvement.
• Act as a coach/mentor to patient care associates and participate in governance.
• May assume charge nurse duties with RN oversight, including rounds and scheduling.
