Chelsea Rehabilitation and Healthcare Center, a 84-bed Sub-Acute and Long-Term Care facility in Virginia, seeks an LPN for a full-time role. You will join a team dedicated to compassionate, patient-centered care and contribute to a supportive work environment.

You'll provide care under the plan of care, administer medications, monitor residents, and document diligently while collaborating with nursing and interdisciplinary teams to uphold high standards of care.

Qualifications

• Active LPN license in Virginia (VA).
• BLS/CPR certification required.
• Ability to manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care per plan of care and physician orders.
• Follow care plans developed by Nurse Manager and interdisciplinary team.
• Administer medications and prescribed treatments per policy.
• Monitor residents and report changes promptly to supervising nurse or physician.
• Maintain accurate, timely documentation.
• Support a safe, respectful environment for residents and families.
