You will review patient records, assess medical, social and emotional factors, and support comprehensive care planning. Qualifications

• LPN license required.
• Minimum 6 months of direct patient care in acute or managed care settings preferred.
• Proficient in patient assessments and monitoring vital signs.
• Excellent communication and interpersonal skills to collaborate with healthcare professionals.
• Experience with geriatric behavioral health preferred.
• Comfort with EMR and computer use; able to lift/move patients and sit for extended periods.

Responsabilités

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management to ensure continuity of care and appropriate resource use.
• Monitor and evaluate patient progress and adjust care plans as needed.
