Job summary

Infinite Medical P.C., in Hollywood, FL, is hiring a Licensed Practical Nurse to review patient charts, verify accuracy, and ensure records reflect treatment requirements. This is an in-office, full-time role with daytime hours.

You will assess medical, social, and emotional needs, provide case management, and collaborate with healthcare teams to optimize resident outcomes. LPN license is required, with 6 months of direct care preferred.

Qualifications

• LPN license required.
• Minimum 6 months direct patient care in acute or managed care setting preferred.
• Proficient in conducting patient assessments and monitoring vital signs.

Responsibilities

• Conduct comprehensive assessments of patients' medical, social, and emotional needs.
• Provide case management to ensure continuity of care and appropriate resource use.
• Monitor and evaluate patient progress and adjust care plans as needed.
• Collaborate with interdisciplinary teams to optimize patient outcomes.
• Oversee patient EMR to ensure proper care.
• Complete reports to reduce patient gaps and hospitalizations.
