Qualifications

• Current unrestricted LPN license in New Jersey.
• Minimum two years of clinical nursing experience.
• Experience in Home Health, Care Management, Case Management, Geriatrics, Community-Based Services, Primary Care, or Assisted Living.
• Valid New Jersey driver's license with reliable transportation.

Responsibilities

• Coordinate services across the healthcare continuum to ensure timely, efficient, and member-centered care.
• Serve as the primary contact for assigned members and caregivers.
• Assist the RN Care Manager with implementation and monitoring of individualized care plans.
• Perform ongoing telephonic and in-person follow-up with members to monitor changes in condition and service needs.
• Coordinate physician appointments, referrals, diagnostic testing, transportation, errands, and community resources.
• Facilitate smooth transitions following hospitalization, emergency department visits, skilled nursing stays, rehabilitation, or hospice services.
• Assist members and families in navigating healthcare services and available FellowshipLIFE programs.
• Identify barriers to care and communicate concerns promptly to the RN Care Manager.
