Job summary

The Lakes at Litchfield, a luxury senior living community in Pawleys Island, SC, seeks a compassionate Resident Care Coordinator with an active SC LPN license to lead clinical operations in Assisted Living and Memory Care.

In this role you will conduct resident assessments, oversee service plans, mentor care staff, partner with providers and families, and ensure regulatory compliance while delivering exceptional quality of life for residents.

Qualifications

• Active South Carolina LPN license required.
• Strong clinical judgment and attention to detail.
• Leads by example and fosters teamwork.
• Effective communication with residents, families, providers and staff.
• Experience in Assisted Living, Memory Care, Long-Term Care, or senior living.
• Thrives in a fast-paced environment and takes initiative.

Responsibilities

• Conduct resident assessments and oversee service plans.
