A healthcare staffing agency is seeking an experienced LPN Care Coordinator in San Francisco, CA. This on-site role focuses on supporting chronic care management through patient outreach, documentation, and care coordination, collaborating with multiple healthcare professionals. Ideal candidates will hold an unrestricted California LPN license and have prior experience in care coordination or telehealth with older adults. This role may transition to remote or hybrid work based on client needs.

Competenze

• Active, unrestricted California LPN license required (multi-state licensure preferred).
• Prior experience in chronic care management, care coordination, or telehealth, particularly with older adult or neurodegenerative populations.
• Proficient with EMRs and remote documentation workflows.

• Provide ongoing care coordination and support for patients in chronic care management.
