Job summary

Baptist Health Arkansas is seeking an LPN Care Coordinator to work with physicians and clinical staff to support patients with chronic conditions and complex needs. This role emphasizes coordinating care according to established guidelines.

The successful candidate will hold an active LPN license in good standing (AR or multi-state with AR), CPR certification, and a driver’s license with reliable transportation. 3–4 years of experience and ambulatory/care management background are preferred.

Qualifications

• Current LPN license in good standing (AR or compact with AR)
• CPR certification required
• High School diploma or GED if working in the lab
• 3–4 years of experience preferred
• Ambulatory and/or care management experience preferred
• Current Driver’s License in good standing with reliable transportation

Working Hours: Monday-Friday8:00 AM -5:00 PM

This job will be authorized80.00 hours bi-weekly.
