Pro Care Innovations is looking for a LPN/LVN to join our team. We are seeking Care Team Coordinators to provide care within the CCM/PCM Service. This role involves delivering at least 20 minutes of care coordination per month for Medicare patients via our telehealth platform. Key responsibilities include telephone interactions with patients, documenting patient interactions, developing care plans, conducting chart reviews, coordinating care among providers and facilities, and working collaboratively to optimize patient outcomes.

This case management position requires an independent work ethic and strong communication skills. An understanding of person-in-environment and systems theory is beneficial for providing compassionate, patient-centered care. As part of our care team, you will help improve patients' quality of life through personalized telephonic case management.

This is a remote position with equipment provided.

Responsibilities:

1. Verifying patient eligibility for PCM/CCM programs
2. Making outbound calls to educate patients and obtain consent for enrollment
3. Providing monthly PCM/CCM services, including care plan support
4. Documenting patient engagement and submitting reports within PCI systems and EMRs

Requirements:

• At least 2 years of healthcare experience, preferably with EMRs
• Self-starter with excellent patient assessment skills
• Understanding of medical terminology
• Strong computer literacy and systems aptitude
• Multitasking skills with effective use of computer and phone
• Excellent verbal and written communication skills
• Interpersonal skills with patients and healthcare providers

Comfortable with outbound calling and enrolling patients into Medicare programs.

Regular office hours are 8:30 am – 5:00 pm CST, Monday through Friday.

Job Type: Full-time, 40 hours/week

Compensation: Paid on commission with a guaranteed hourly wage ($17.00 - $19.00 based on experience), with opportunities for bonuses and PTO. Income is uncapped based on patient volume.
