Position Summary
North Canton Nursing & Rehab is seeking dedicated Licensed Practical Nurses (LPNs) to join our clinical team across all shifts. In this role, you will provide high-quality direct nursing care, administer medications and treatments, monitor resident health status, and collaborate with our interdisciplinary care team to ensure individualized, compassionate care for our long-term care and short-term rehabilitation residents.
Key Responsibilities
Direct Resident Care: Administer medications, perform prescribed wound care, carry out routine clinical treatments, and assess resident conditions in accordance with care plans and physician orders.
Clinical Monitoring & Documentation: Maintain accurate, timely charting in the Electronic Health Record (EHR) system; document vital signs, care updates, and any changes in physical or mental status.
Care Coordination: Collaborate closely with Registered Nurses (RNs), Physicians, MDS Coordinators, and therapists to implement and update individualized care plans.
Team Leadership: Direct and supervise State Tested Nursing Assistants (STNAs) on shift to ensure daily personal care tasks and safety protocols are met.
Emergency & Change of Condition: Promptly recognize, report, and initiate appropriate clinical interventions for acute changes in resident conditions or medical emergencies.
Regulatory Compliance: Maintain strict adherence to Ohio Department of Health regulations, facility policies, and infection control procedures.
Qualifications & Requirements
Education & Credentials: Valid, unencumbered Licensed Practical Nurse (LPN) license in the State of Ohio.
Experience: Minimum 1 year of clinical nursing experience in a skilled nursing, long-term care, or rehabilitation environment preferred.
Certifications: Active CPR / BLS Certification required.
