Job summary

The University of Vermont Health Network Home Health & Hospice in Colchester, VT seeks a compassionate LPN to deliver skilled nursing care in patients' homes under RN supervision.

You will assess needs, administer meds, educate families, and collaborate with a multidisciplinary team. A Vermont LPN license and 2+ years of home health/hospice experience are required; generous sign-on and relocation packages are available for full-time roles.

• Two or more years of directly related home/public health, hospice and/or maternal/child health nursing experience.
• Strong assessment, communication, and interpersonal skills.
