Job summary

Hospice of Central PA is seeking an LPN Admissions Representative for a day shift. This PRN, on-site role handles preadmission discussions, EMR data entry, triage of admissions, and arranging post‑discharge visits to meet patient needs.

The candidate will be a graduate of an approved practical nursing program, hold a Pennsylvania license, and have at least one year of recent LPN experience. Availability to cover visits based on staffing is essential.

Qualifications

• Graduate of an approved practical nursing school.
• Maintain a current Pennsylvania nursing license.
• Have at least one year of experience as a licensed practical nurse within the last four years.

Responsibilities

• Conduct remote preadmission discussions with patients and families to capture and document patient story and schedule visits based on level of complexity and urgency.
• Enter nursing‑related information into the EMR, including allergies and pertinent diagnoses.
• Triage phone calls to answer nursing‑related questions and prioritize admission visits.
• Make follow‑up calls and conduct remote chart reviews for hospitalized patients awaiting discharge and ensure post‑discharge arrangements are prepared.
• Support admissions nurses by ordering equipment, supplies, and medications.
• Be available to cover admission and follow‑up visits based on staffing and referral volumes.

LPN Admissions Representative

Shift: Day Shift (8am - 5pm or 9am - 6pm)

Employment Type: PRN – as needed

Not a remote position.

Essential Functions

• Conduct remote preadmission discussions with patients and families to capture and document patient story and schedule visits based on level of complexity and urgency.
• Enter nursing‑related information into the electronic medical record (EMR), including allergies and pertinent diagnoses.
• Triage phone calls coming into the Intake & Admissions Office to answer nursing‑related questions and prioritize admission visits so patients with the greatest needs are escalated.
• Make follow‑up calls and conduct remote chart reviews for hospitalized patients awaiting discharge, ensuring all arrangements for the post‑discharge visit are prepared (medications, durable medical equipment, transportation, etc.).
• Support admissions nurses by ordering equipment, supplies, and medications.
• Be available to cover admission and follow‑up visits based on staffing and referral volumes.
