The role emphasizes geriatric and memory care, with responsibilities spanning assessments, treatments, and patient education for discharge planning and optimal recovery.

Qualifications

• Proven experience with geriatric populations, memory care, or developmental disabilities.
• Strong knowledge of medical terminology, anatomy, physiology, and ICD-9/10 coding.
• Experience in ICU or trauma center environments; Level I or II trauma desirable.
• Skilled in emergency procedures including airway management, first aid, CPR/BLS, and ventilator management.
• Familiar with EMR/EHR systems and medical documentation.
• Ability to perform venipuncture, phlebotomy, injections, catheter care, and sterile technique.
• Knowledge of long-term care, hospice care principles, pain management, and infection control.

Responsibilities

• Provide direct patient care, including assessments and vital signs.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Administer treatments such as IV infusions, catheterizations, wound care.
• Document medical records accurately using EHR systems.
• Monitor patients, evaluate responses to treatments, and support clinical decisions.
• Assist with diagnostic evaluations and radiology procedures when necessary.
• Educate patients and families on health management, medications, and discharge planning.
