Job summary

Carle Health is seeking a Licensed Practical Nurse in Illinois to support RN-led patient care. The LPN will administer medications, manage IV piggybacks, and assist with assessments and care plans after the initial RN evaluation is completed.

Duties include trach care, NG tube care, ostomy care, and patient education. Responsibilities emphasize collaboration with physicians/APPs, charting, and continuity of care, with emphasis on discharge instructions and adherence to HIP and Knee replacement

Qualifications

• Education: LPN program specified.
• ACLS within 1 year per AHA.
• BLS within 30 days per AHA.

Responsibilities

• Passes all medications and hangs IV piggybacks and fluids.
• Charts assessments on assigned patients after the RN's initial assessment.
• Charts on care plans opened by the RN; assist with new care plans as needed.
• Coordinate care with physicians/APPs to receive orders; assist with dressing changes as needed.
• Complete trach care and care for NG tube and feedings.
• Care for and change ostomy; reinforce discharge instructions.
