Job summary

Hireology is seeking a Case Manager in Virginia to support home health patient care and nursing visits in accordance with the plan of care.

The ideal candidate holds an LPN or RN license from the Virginia Board of Nursing and has direct patient care experience in a home care setting. Experience in home health and reliable, dedicated performance are required.

Qualifications

• Must hold a current license as a Licensed Practical Nurse or Registered Nurse from the Virginia Board of Nursing.
• Must have experience working in a home care agency.
• Must have direct patient care experience.

Responsibilities

• Complete patient care duties.
• Follow patient plan of care and reevaluate as necessary.
• Work scheduled shift to perform patient duties.
• Complete clinical nurse notes in their entirety.
• Complete skilled care services as applicable.

Case Managers perform tasks such as medication management, wound care, vital sign monitoring, and patient education in accordance with the established plan of care.
