Job summary

Home Health Services of Virginia is seeking dedicated Case Managers who are licensed RNs or LPNs and have hands-on experience in home health care.

You will assess patients, coordinate skilled nursing visits, monitor vitals, educate patients, and ensure adherence to the plan of care in a fast-paced home health setting in Midlothian, Virginia. Strong communication with families, physicians, and caregivers is essential, and you must follow regulatory standards set by the Virginia Board of Nursing.

Qualifications

• Must hold a current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) from the Virginia Board of Nursing.
• Must have experience working in a home care agency.
• Must have direct patient care experience.

Responsibilities

• Complete patient care duties.
• Follow patient plan of care and reevaluate as necessary.
• Work scheduled shift to perform patient duties.
• Complete clinical nurse notes in their entirety.
• Complete skilled care services as applicable.

Job description

Case Managers are a critical part of the agency and must have experience working in the home health setting. Case Managers facilitate direct patient care and skilled nursing visits. Case Managers perform tasks such as medication management, wound care, vital sign monitoring, and patient education in accordance with the established plan of care.
