Job summary

Outcome HC in Philadelphia is seeking licensed practical nurses to join our team of dedicated health care professionals. The ideal candidate will provide daily bedside care, monitor vital signs, administer medications, record patient information, and support residents in long-term or skilled nursing settings while upholding compassion, teamwork, and professional standards.

A Pennsylvania LPN license and familiarity with PCC are required; prior long-term care experience is preferred.

Qualifications

• LPN license in Pennsylvania is required.
• Successful completion of the LPN license program.
• Long-Term/Skilled Nursing Facility experience preferred.
• Familiarity with PCC software.

Responsibilities

• Record a patient’s medical history accurately.
• Take and record measurements of blood pressure, temperature, heart rate, etc.
• Observe patients under treatment to identify progress and side-effects.
• Monitor patients’ condition and compose patient charts.
• Assist patients with daily activities such as eating and bathing.
• Administer injections, prescribed medications, and wound care.
• Collect samples for lab testing as assigned.
• Provide emotional and psychological support when needed.
• Communicate with patients’ family or friends with guidance.

We are looking to invite reliable and dedicated Licensed Practical Nurses, LPN to join our team of dedicated health care profressionals.

The ideal candidate would be a nurse that is looking to provide outstanding patient care and physical support for patients and residents on a daily basis.

To do well in this role you should have your Practical Nursing license in Pennylvania.

Responsibilities

• Record a patient’s medical history accurately
• Take and record measurements of blood pressure, temperature, heart rate etc.
• Observe patients under treatment to identify progress, side-effects of medications etc.
• Monitor patients’ condition including fluid intake and output and compose patient charts
• Assist ailing patients in daily necessary activities such as eating, bathing etc.
• Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
• Collect samples for lab testing as assigned
• Provide emotional and psychological support when needed
• Communicate with patients’ family or friends to provide advice, comfort and release instructions

Qualifications

• LPN LICENSE IN THE STATE OF Pennsylvania!
• Successful completion of LPN license program is a must!
• Long-Term/ Skilled Nursing Facility Experience
• Familiarity with PCC
• Compassionate and able to handle stress
