Job summary

Outcome HC is seeking Licensed Practical Nurses (LPN) in New Jersey to join a dedicated healthcare team. You will provide daily patient care and physical support for residents in a long-term care setting.

Ideal candidates hold a New Jersey LPN license, have completed an LPN program, and bring experience from long-term or skilled nursing facilities. PCC familiarity and strong teamwork are desired, with a $1,000 sign-on bonus offered.

Qualifications

• Current LPN license in New Jersey.
• Successful completion of LPN license program is a must.
• Experience in Long-Term/Skilled Nursing facilities.
• Familiarity with PCC.

Responsibilities

• Record a patient’s medical history accurately.
• Take and record measurements of blood pressure, temperature, heart rate, etc.
• Observe progress and side-effects of medications.
• Monitor patient condition and maintain charts.
• Assist patients in daily activities (eating, bathing, etc.).
• Administer injections and prescribed medications; assist with wounds.
• Collect samples for lab testing as assigned.
• Provide emotional support and communicate with families.

Come join us! We are looking to invite reliable and dedicated Licensed Practical Nurses, LPN to join our team of dedicated health care profressionals.

The ideal candidate would be a nurse that is looking to provide outstanding patient care and physical support for patients and residents on a daily basis.

To do well in this role you should have your Practical Nursing license in New Jersey

Responsibilities

• Record a patient’s medical history accurately
• Take and record measurements of blood pressure, temperature, heart rate etc.
• Observe patients under treatment to identify progress, side-effects of medications etc.
• Monitor patients’ condition including fluid intake and output and compose patient charts
• Assist ailing patients in daily necessary activities such as eating, bathing etc.
• Administer injections, prescribed medications, enemas etc., clean and dress wounds and assist with other basic medical care tasks
• Collect samples for lab testing as assigned
• Provide emotional and psychological support when needed.
• Communicate with patients’ family or friends to provide advice, comfort and release instructions

Qualifications

• LPN LICENSE IN THE STATE OF New Jersey!
• Successful completion of LPN license program is a must!
• Long-Term/ Skilled Nursing Facility Experience
• Familiarity with PCC
• Compassionate and able to handle stress
