Job summary

Kadima Rehabilitation & Nursing at Cheswick is seeking a dedicated Licensed Practical Nurse (LPN) to provide direct patient care in accordance with physician orders and established care plans. Join an interdisciplinary team to support residents’ physical and emotional well-being.

Qualifications

• Active Pennsylvania LPN license or eligibility.
• Experience in long-term care or skilled nursing preferred but not required.
• Strong clinical, communication, and organizational skills.
• Compassionate, patient-centered approach to care.

Responsibilities

• Assess residents’ health status through observation, physical assessment, and review of diagnostic information
• Administer medications and treatments in accordance with physician orders
• Participate in care planning and collaborate with the interdisciplinary team
• Monitor, document, and communicate changes in resident condition to physicians and nursing leadership
• Supervise and support nursing assistants and other care staff
• Educate residents and families on health conditions and care plans
• Complete required documentation accurately and in compliance with regulations and facility standards
