Job summary

EOSIS Bridge in Sauk Rapids, MN seeks a dedicated Licensed Practical Nurse (LPN) for a full-time, 3rd shift role to support individuals in recovery. You’ll work under supervision, providing licensed care and contributing to a supportive team environment.

This role offers $27–$30/hr with a $5,000 sign-on bonus, 20 PTO days, 6 holidays, comprehensive insurance, tuition reimbursement, 401(k) options, and clear pathways for career growth within EOSIS Bridge.

Qualifications

• LPN license valid in Minnesota.
• CPR and First Aid certification required.
• Minimum 1 year in a chemical dependency facility; 2–3 years preferred.
• Document 12 hours of co-occurring mental health and chemical dependency training.
• Know and follow Minnesota Department of Health regulations.

Responsibilities

• Adhere to facility policies and procedures.
• Provide nursing services and education to clients.
• Observe and report symptoms or changes in client condition.
• Administer Mantoux test upon admission.
• Complete weekly updates and chart audits.
• Maintain compliance with health regulations.
