Green Hill Senior Living & Rehabilitation in West Orange, NJ is seeking a qualified LPN to join our nursing team and deliver exceptional patient care. You will supervise CNAs and provide individualized nursing care according to each resident’s plan, while supporting admissions, transfers, and discharges.

Responsibilities include administering medications, explaining procedures to residents, monitoring vital signs, and maintaining accurate care records.

Qualifications

• Active, unencumbered LPN licensure in New Jersey.
• Excellent communication and interpersonal skills.
• Highly organized and detail oriented.
• Ability to work independently or as part of a team.

• Explain procedures and treatments ordered to gain patients’ cooperation and alleviate apprehension.
• Observe vital signs to detect deviations from normal and gauge progress.
• Maintain patients’ medical records on nursing observations and actions taken.
• Record nursing needs of patients on care plans to ensure continuity of care.
• Observe emotional stability of patients and prepare them for continuing care after discharge.
• Perform other duties as required to provide exceptional care.

What You'll Do
