Job summary

Northwest Nursing and Rehab Center in Baltimore, MD is seeking compassionate Licensed Practical Nurses (LPNs) to provide high-quality nursing care to our residents. This role involves 12-hour shifts and collaboration with RNs to implement individualized care plans.

About the Role:

As an LPN on our team, you will have the opportunity to work 12-hour shifts, providing personalized care to our residents. Your primary responsibilities will include:

• Administering medications and treatments as ordered by physicians
• Monitoring resident health and reporting changes to the care team
• Collaborating with Registered Nurses (RNs) and other care team members to develop and implement individualized care plans
• Maintaining accurate and detailed documentation of resident care
